The serpentine helm is a degradable helmet made at level 52 crafting by using a chisel on a serpentine visage, giving the player 120 Crafting experience. Its bonuses are comparable to those of the Barrows helmets. However, unlike the Helm of neitiznot and Verac's helm, the serpentine helm offers no prayer bonus. The helmet requires 75 Defence to wear.

The helmet is charged with Zulrah's scales, requiring 11,000 scales to fully charge it, costing 2,277,000. It cannot be equipped if it is uncharged, except when fully degraded while equipped. Once charged the helmet will last for 15 hours of combat, before returning to its uncharged state. It has the highest strength bonus in the head slot, making it one of the best helmets in the game.

When equipped, the helmet will grant immunity to venom and poison. In addition, if an opponent attacks a player with the helmet equipped, there is a 1 in 6 chance (16.7%) the opponent will be inflicted with venom, providing that they are not immune to poison. The opponent does not have to deal damage to the player in order to be inflicted with venom.

If a player dies with the helmet, any scales that were used to charge it will appear on the floor along with the uncharged helmet.

Several bosses, and many high level slayer monsters are immune to poison, and thus are immune to venom as well. However, some bosses, such as the Dagannoth Kings and TzTok-Jad, are immune to venom only; the helmet will inflict normal poison damage on them.

Players can dismantle an uncharged helm to receive 20,000 Zulrah's scales, resulting in a loss of −25,583 coins.

The serpentine helm also has two variations of recolors, one being the magma helm, requiring a magma mutagen, and the other being the tanzanite helm, requiring a tanzanite mutagen. The only way to obtain the mutagens is by killing Zulrah itself as the mutagens cannot be traded.
